Payments must be made before we will start your request. 

Payments can be made via the phone or in person at our front desk. Card payments only. 

Time Scales

Our minimum turnaround time for non-NHS work is 20 working days and may take longer as our GPs complete the work in their own time.

Requests are only actioned after payment has been received.

Subject Access Requests

A SAR is a request that can be made in writing or by asking for access to the personal information a company or organisation holds on you. This is a legal right that any individual in the UK is entitled to exercise at any point for free. 

You can ask someone else to submit a subject access request for you, for example, a solicitor or family member acting on your behalf. You may be asked to provide confirmation that you have asked them to do this.

Whilst a SAR gives you the right to obtain a copy of your personal data, it should be noted that there are other ways to obtain your health and care records. The NHS is seeking to empower people and transform their experience of health and care by giving them the ability to access, manage and contribute to digital tools, information, and services - for example, most patients can now request access to their GP record online, including via the NHS App. More information on this can be found on our patient record page. 

To request a subject access request, please email or write to us and include:

  • Name
  • DOB
  • What personal information you want (be specific about the information you’re asking for, and where relevant, say what information you don’t need)
  • Details or dates that will help the organisation find the information you want

 

IGPR act on behalf of Park Medical Centre for some certain types of subject access requests and insurance reports

The report will be sent to you or your third party for access by a secure download link.
