How to Register with Park Medical Centre in Chester

You can obtain a registration pack, which contains all the information and forms you need in order to register, from our receptionists. If several members of your family want to register, you will need one set of forms for each family member.

Once you've completed all the forms, you'll need to bring them into the surgery together with your proof of identity. (A list of acceptable proof is attached below).
Once we have received your completed forms, and before seeing a doctor, we’ll make an appointment for your new patient health check, which is carried out by a member of the nursing team. It’s important that you attend this appointment so that we can give you a general health check and discuss any health issues highlighted on your medical questionnaire.
Proof of Identity Required for Registration Purposes
When you register with the practice, you will need to provide the following proof of your identity/address
